We’ve been here before. Initial reports of an incident grow quickly into something much more horrific, with the media struggling to make sense of the fast moving chaos. And just like Aurora, Newtown, and the Boston bombings, the coverage of Monday’s mass shooting at the Washington Navy Yard had notable mistakes.
